2015-2016 Registration Notice
Registration for the 2015-2016 school year is Thursday, July 30th from 1-4p.m. and 6-8p.m. and Friday, July 31st from 9a.m.-12p.m., in the Library (IMC).
In the near future, before registration, you will receive a packet in the mail with documents that are very important to our district. Please fill out this information and turn it in to the school on or before registration. Everyone will receive a new emergency form to fill out, which will allow us to update our records. It is extremely important this form be updated each year so we can contact parents or guardians in case of an emergency. The other paperwork needs to be received on or before the registration dates so we can fill out the necessary reports and update all of our information before the new school year begins. Please note every incoming freshman student needs a school physical, a Td (tetanus) booster if they have not had one in the last ten years, and two doses of the Varicella (chickenpox) vaccine.
Incoming kindergarten students need a school physical, eye exam, dental exam, and up-to-date immunizations. Students entering 2nd grade need a dental and eye exam and students entering 6th grade need a school physical and a dental exam.
New this year, any child entering sixth grade shall show proof of having received one dose of meningococcal conjugate vaccine on or after the 11th birthday. Also, any child entering 12th grade shall show proof of having received two doses of meningococcal conjugate vaccine prior to entering the 12th grade. The first dose shall have been received on or after the 11th birthday, and the second dose shall have been received on or after the 16th birthday, at least eight weeks after the first dose. If the first dose is administered when the child is 16 years of age or older, only one dose is required.
Please make all necessary medical appointments early as the state has a deadline in October for all students to be in compliance with all immunization records.
If your child is interested in playing sports (5th-12th grade), he or she will need a sports physical that can be obtained during a regular school physical. The sports physical must be turned in before any student can participate in any sport’s tryouts or practices as no exceptions will be made. Also, state law and IHSA rules require all students to be covered by an accident/ health insurance policy in order to participate in high school sports. Please fill out the insurance form included in your registration packet and attach a copy of your child’s insurance card. If you do not have a copy of the insurance card, you can make a photocopy at registration. All student-athletes (grades 5-12) should complete this insurance verification process.
At registration, you will receive an updated student handbook and other pertinent information for the upcoming school year. High school students who drive to school will need a parking card on file in the junior high /high school office. We will distribute parking permits to students once they have been assigned, which will be a few days after school starts. Also, junior high and high school students will have the opportunity to purchase new physical education uniforms, if needed.
